Assistance offered by APPOINTMENT ONLY

FINANCIAL ASSISTANCE FOR CLIENTS WITH MEMBERSHIP CARDS:

***IF YOUR MEMBERSHIP CARD HAS EXPIRED, PLEASE FILL OUT THE FINANCIAL REQUEST FORM FIRST***

Medication Assistance – does not cover controlled medications:

  • Please call the Co-Op and leave a message requesting medication.
  • Please leave your name, member number, the name and phone number for the pharmacy.
  • We will call you when your check is ready.

Utility Assistance:

  • Complete the financial assistance request form.
  • A staff member will contact you to go over your request.  Please be sure to list your correct phone number and email.  We will attempt to reach you by phone first.  If we can not reach you by phone, we will email you.
  • Due to the high volume of request, please allow 3 business days for a response.  Please be sure to check your voicemail and/or email on a regular basis.
  • DO NOT FILL OUT MULTIPLE REQUESTS.

Housing Assistance:   Not Available

  • DO NOT FILL OUT MULTIPLE REQUESTS.
  • Requested documentation must be submitted within 5 days of the request.  If information is not received, your request will be closed.

Documents required for financial assistance:

  1. Photo IDs for all adults in the home.
  2. Social Security cards (or proof of social security number) for all adults in home.
  3. Birth certificates or social security cards for all children in home.
  4. Proof of ALL household income.
  5. Bank statements for last two months (with account numbers blacked-out).
  6. Current utility bill.
  7. First page of current lease or mortgage statement.
  8. Documentation of unemployment (if applicable).
  9. Documentation of past due balance on the account you are seeking financial assistance.

*Other documentation may be required.
